While dodgeball is safe, the nice people at the insurance agency request that everyone signs a waiver. So, each season, every 604dodgeball participant must read and sign our waiver to participate in activities at any of our locations. If we do not receive a waiver from you, you are not permitted to play.
Your privacy is important to us! The information you provide to us will only be used to communicate information about 604dodgeball, such as news, schedules, and other important information.
You are required to download one via the link and bring the completed form on your first day of dodgeball. If you are a drop in, please bring it with you and hand it to any 604dodgeball staff. Please also ensure you fill out the correct season information! Ie. Fall 2009, Winter 2010
